DATA WE COLLECT

Sedgwick collects information through the Site on behalf of New Hampshire Insurance Company related to certain insurance products offered by New Hampshire Insurance Company through Mastercard International Incorporated. Information collected may include without limitation personally identifiable information such as name, address, telephone number, e-mail address, social security number, account number and information related to medical history or treatment. Providing your personal information is optional however failure to provide the information requested may result in a delay in the processing of your claim/request.

Any insurance claim filed pursuant to your Mastercard sponsored insurance program on this website will be evaluated by New Hampshire Insurance Company, Sedgwick, or their authorized local representatives and, if accepted, will be paid by New Hampshire Insurance Company or by Sedgwick on behalf of New Hampshire Insurance Company.
